Are these users seeing the access denied page are newly registered? Are 
the rules sticky?
Did you enable any caching under performance settings, are you using 
memcache, or something like that?

For the D7, I have tried both, the latest stable version 7.x-4.0 and the 
dev version 7.x-4x-dev. I am running a VPS on a Debian OS. This happens 
only on certain roles especially the ones specified in Shibboleth rules 
integration. If I create a new role, everything works fine, when i 
assign that role using rules in Shibboleth rules then all users with 
that role get access denied on the first redirection.
I have also tried to print out the $user object during debugging and it 
seems that when the access is denied, the Session variable is not 
present in the user object.
Once I refresh the page, the session object comes up. Did you have the 
same situation?

Are you using the latest version of shib_auth on both sites? We had 
similar issues before, but shouldn't have on the latest version / dev. 
What kind of server does your site runs on?
