[shib_auth] Access Denied error message for pages

"Here and there" is not very specific. Most common error is when the user
navigates away from Shibboleth-protected locations, thus the headers are not
populated -> roles are not assigned -> access is denied.

You can trace requests with LiveHTTPHeaders Firefox extension, or look for the
headers with debug mode on.
