[Bonetools] unknown worked bone artefact from Italy

Dear colleagues,
I am currently working on a medieval (late 12th century) faunal  
assemblage from central Italy. Among the remains it was found the  
beautiful worked bone you can see in the attached photos. I send  
several views. It is 56,5 mm long and I guess it was manufactured from  
a middle sized mammal tibia. Any suggestions on its functionality or  
parallels are most welcome.
